Safeguarding Adult Reviews and Domestic Abuse Related Death Reviews

The Wokingham Borough Safer Communities Partnership (WBSCP) works with local partners to keep people safe, support those at risk of harm, and learn when serious incidents or deaths occur.

This page brings together published statutory reviews relating to:

  • Safeguarding Adult Reviews (SARs), carried out under section 44 of the Care Act 2014 through the West of Berkshire Safeguarding Adults Board, and
  • Domestic Abuse Related Death Reviews (DARDRs), previously known as Domestic Homicide Reviews, commissioned under section 9 of the Domestic Violence, Crime and Victims Act 2004, and updated in line with the Domestic Abuse Act 2021.

These reviews are undertaken when an adult has died or been seriously harmed and there are concerns about how organisations worked individually or together to provide support and protection. Their purpose is to identify learning and improve practice. They are not about blame.

We recognise that each review relates to a real person, and that these events can have a lasting impact on families, friends, professionals and communities. We acknowledge this with respect and extend our sincere condolences to all those affected.

Why these reviews are published

Publishing these reviews helps to:

  • improve how organisations work together to safeguard adults
  • strengthen professional practice and partnership arrangements
  • identify opportunities for earlier help and prevention
  • reduce the risk of similar harm happening again

The term Domestic Abuse Related Death Review (DARDR) reflects national changes recognising that deaths linked to domestic abuse may include suicide and deaths associated with coercive, controlling, emotional or economic abuse, as well as physical violence.
